Wearing soft lenses should feel effortless. When you wake up to a red, painful eye that can’t face the light, a contact lens-associated peripheral ulcer (CLPU) could be to blame. Although this condition usually heals without lasting vision loss, it does demand quick action.
In the next few minutes, you’ll learn what CLPU is, common causes, how to recognize the problem early, and smart CLPU management habits that keep trouble from returning.
What Is Contact Lens-Associated Peripheral Ulcer (CLPU)?
Contact Lens-Associated Peripheral Ulcer (CLPU) is a shallow, round or crescent-shaped ulcer that forms near the edge of the cornea. It’s usually sterile—no live bacteria tunnel into the tissue—but the cornea still mounts a painful inflammatory response.
CLPU typically only affects one eye, heals quickly with care, and leaves only a faint peripheral scar that seldom affects vision.
What Causes CLPU?
A handful of risk factors stack up to spark an ulcer. Keep an eye on these common CLPU causes:
- Mechanical irritation from tight, aging, or poorly fitted lenses that scrape the peripheral cornea
- Low-oxygen wear caused by over-wearing lenses or choosing materials that block airflow
- Bacterial toxins—especially from Staphylococcus species—that build up on lenses, cases, or eyelids when hygiene slips
- Closed-eye environment during overnight wear, which traps heat, debris, and bacteria under the lens
- Lid margin disease, such as blepharitis, which adds more bacteria to the tear film
While no brand is singled out for contact lens-associated peripheral ulcer (CLPU), certain materials and wear schedules raise the odds.
Silicone-hydrogel lenses worn overnight or for “extended wear” show the highest CLPU rate. Studies place the incidence around 2–3 % in daily wear and even higher when the same high-oxygen lenses stay in during sleep. The stiff (high-modulus) nature of some silicone-hydrogel designs can rub the peripheral cornea, and the closed-eye environment traps bacterial toxins. Older hydrogel lenses worn past their scheduled replacement or slept in also draw more cases, largely because low oxygen and heavier deposit build-up irritate the cornea.
Rigid gas-permeable (RGP) lenses and modern daily disposables, like the Acuvue Oasys 1-day for Astigmatism contact lenses, sit at the low end of CLPU risk. RGPs move more with each blink, flushing debris, while daily disposables start clean every morning and come out before bed.

How Do You Spot CLPU Early? Signs & Symptoms
CLPU usually strikes overnight. You might feel a gritty “sand-in-the-eye” sensation followed by sharp, localized pain and light sensitivity. The white of the eye turns red mainly around the lesion, and vision can blur slightly if swelling spreads.
In the clinic, your eye-care professional sees a tiny gray-white spot near the colored iris that stains with fluorescein dye. Early diagnosis is important because early care speeds healing and rules out a true infection.
CLPU Treatment: How Do Doctors Handle It?
CLPU treatment always starts with a lens break—no contacts until the cornea fully re-epithelializes. Your clinician then prescribes a broad-spectrum antibiotic drop to cut surface bacteria and may add a cycloplegic drop to ease light sensitivity.
Most ulcers shrink within 24–48 hours. Once the surface closes, a mild steroid drop can calm lingering inflammation, but only under professional supervision. Follow-up visits will confirm that the ulcer is shrinking and that no infectious keratitis is hiding beneath.
CLPU Management Tips at Home and During Follow-Up
Good medical care works best when paired with smart CLPU management at home:
- Stick to the drop schedule. Set phone alarms so every dose lands on time.
- Discard the old lenses, case, and solution. Starting fresh eliminates hidden toxins.
- Upgrade to daily disposables or high-oxygen silicone-hydrogel lenses when you return to wearing them.
- Clean the eyelids each night with warm water or a lid-scrub pad to lower bacterial load.
- Keep every follow-up appointment until your eye-care professional gives an all-clear.
Wrap-Up: Keep CLPU Off Your Radar
Fast action keeps a contact lens-associated peripheral ulcer from leaving a mark. If you experience these symptoms, remove the lens, book an exam, follow the drop plan, and keep lids and lenses squeaky-clean.
Authentic lenses matter, too. Counterfeit or mishandled stock can carry deposits, low-oxygen materials, or hidden microbes that raise the risk of CLPU. Stick with trusted suppliers such as Lens.com, which ships factory-sealed boxes straight from the manufacturer and verifies every prescription.
