Wearing contact lenses should feel hassle-free and comfortable. But if you experience stinging, burning, or you become unusually sensitive to the light, there could be more going on than simple eye dryness. One possible cause is superficial punctate keratitis (SPK)—a condition that affects the outermost layer of the cornea.
SPK can sound concerning, but it is common and treatable with the right care. Learning the causes, symptoms, and treatment options can help you manage irritation more effectively, while comparing options like the best daily contacts for dry eyes may help dryness-prone wearers discuss more comfortable lens choices with their eye care professional.
What Is Superficial Punctate Keratitis?
Superficial punctate keratitis (SPK) is a type of eye condition where small lesions develop on the epithelium, which is the thin, outer layer of the cornea. These tiny spots are often invisible to the naked eye, and they are scattered throughout the eye area. Even these are small spots, they still can cause noticeable discomfort and vision disturbance.
For contact lens wearers, SPK can make even normal lens wear feel uncomfortable. Finding the most comfortable contact lenses starts with understanding whether irritation is related to dryness, lens fit, overwear, hygiene habits, or corneal surface changes like SPK.
This condition is typically diagnosed with fluorescein dye during a slit-lamp eye exam, where the lesions appear as bright green dots under blue light.
Superficial Punctate Keratitis Symptoms
The severity of SPK can differ from person to person. You might experience just mild irritation or symptoms that affect your day-to-day life. Here are some usual superficial punctate keratitis symptoms.
- Burning or stinging sensation
- Light sensitivity (photophobia)
- Watery or teary eyes
- Gritty or foreign body sensation
- Blurry vision
- Redness or inflammation
If you’re a contact lens wearer, these symptoms may feel more intense during or after wear.
What Causes SPK?
There are several known superficial punctate keratitis causes, and many of the causes are linked to daily habits or environmental stress.
- Overuse of contact lenses or wearing lenses for too long
- Dry eyes or tear film instability
- UV exposure without eye protection
- Chemical irritants (e.g. chlorine, smoke, or air pollution)
- Infections (viral or bacterial)
- Eye allergies or inflammation
- Post-surgery effects (like after LASIK or cataract procedures)
Superficial Punctate Keratitis Treatment
Fortunately, SPK is temporary and treatable with the right care. These treatments focus on reducing inflammation, promoting healing, and avoiding the underlying cause.
Some SPK treatments are:
- Lubricating eye drops or artificial tears to restore moisture
- Temporary discontinuation of contact lenses
- Antibiotic or steroid eye drops if infection or inflammation is present
- Switching to daily disposable contacts to reduce irritants. Acuvue Oasys 1 Day with HydraLuxe contact lenses are a great example, offering high moisture retention and a smooth, comfortable feel
- Protective eyewear to shield from UV or harsh environments
Plus, almost all cases improve within a few days to a week.
SPK Prevention Tips for Contact Lens Users
If you’re susceptible to eye irritation, prevention is necessary. Here’s how you can reduce your risk of developing SPK:
- Avoid sleeping in your lenses unless prescribed
- Use prescribed lens care solutions. Never tap water
- Give your eyes regular breaks from contacts
- Wear sunglasses with UV protection
- Stick to a proper replacement schedule
