General Steps

1. Purchase the required number of boxes of ACUVUE Brand Contact Lenses within 90 days of your eye exam. See Display above for specific requirements for this ACUVUE product.
   
   
2. To receive a Rebate form visit www.acuvue.com/rebates and download the form.
   
   
3. Fill out the rebate form and follow its instructions.
   
   
4. Mail by 1/31/14 to:
