Why Do They Form?

These cataracts are typically congenital or develop in early childhood. They often occur because of a brief "insult" to the lens during development, such as a metabolic imbalance or maternal infection.

Visual Appearance

Under a slit-lamp exam, a zonular cataract often shows "riders," small, linear opacities that extend from the cloudy layer toward the edges of the lens.

Impact on Development

If the cloudy layer is dense enough to block the central visual axis, it can cause "amblyopia" (lazy eye), where the brain doesn't learn to see through that eye properly.

Treatment Options

If the cataract is small, it may simply be monitored. However, if it hinders visual development, surgery is required to remove the cloudy lens and replace it with an IOL.

Is it different from a "regular" cataract?

Yes. Regular age-related cataracts usually cloud the whole lens over time, whereas zonular cataracts are layered and often appear in children.

Is it always in both eyes?

It is frequently bilateral (in both eyes), especially when the cause is genetic or due to a systemic issue during fetal development.

Will my child need surgery?

Only if the cataract is large or thick enough to prevent the eye from developing clear vision. Many small zonular cataracts never require surgery.
