How Do the Three Inhaled Medicines Work?

Budesonide reduces airway inflammation through glucocorticoid receptor activity. Glycopyrrolate blocks muscarinic receptors involved in airway smooth muscle contraction. Formoterol stimulates beta2-adrenergic receptors and relaxes airway smooth muscle. The three components combine an inhaled corticosteroid with two distinct forms of long-acting bronchodilation.

Who Uses Budesonide, Glycopyrrolate, and Formoterol?

This inhaler is approved for maintenance treatment of chronic obstructive pulmonary disease in adults and for maintenance treatment of asthma in adults and adolescents age 12 and older. It is not indicated for rapid relief of acute bronchospasm. Sudden breathing symptoms are treated with a short-acting bronchodilator prescribed for rescue use.

How Is the Triple Inhaler Dosed?

Adults with chronic obstructive pulmonary disease use two inhalations of 160 mcg budesonide, 9 mcg glycopyrrolate, and 4.8 mcg formoterol twice daily. Asthma in patients age 12 and older uses two inhalations of 160 mcg, 18 mcg, and 4.8 mcg twice daily. The mouth is rinsed with water without swallowing after each dose.

What Safety Problems Need Attention With the Triple Inhaler?

Risks include oral candidiasis, pneumonia, adrenal effects, reduced bone mineral density, paradoxical bronchospasm, cardiovascular effects, cataracts, glaucoma, urinary retention, low potassium, and high blood glucose. Inhaled corticosteroids can reduce growth velocity in pediatric patients. People with narrow-angle glaucoma or urinary retention need added clinical monitoring during treatment.
