What Is Rubeosis Iridis?

Rubeosis iridis is the growth of fine, abnormal new blood vessels on the surface and at the angle of the iris. These vessels usually develop when the retina is chronically starved of oxygen, as in proliferative diabetic retinopathy or ischemic retinal vein occlusion. The ischemic retina releases growth factors that circulate forward and stimulate iris neovascularization. Early rubeosis can be subtle, but as it progresses, the vessels become more obvious and fragile. Without treatment, this process can lead to severe secondary glaucoma and vision loss.

Causes and Pathophysiology

Long standing poorly controlled diabetes is a major cause, especially when retinal ischemia triggers widespread neovascularization. Central retinal vein occlusion, ocular ischemic syndrome from carotid artery disease, and chronic retinal detachment are other important triggers. The new vessels grow across the iris and into the drainage angle, accompanied by fibrous tissue. Contraction of this fibrovascular membrane can close the trabecular meshwork and pull the iris forward. The result is a sharp rise in intraocular pressure known as neovascular glaucoma.

Clinical Features and Diagnosis

Patients may have few symptoms early on, especially if retinal disease was already reducing vision. As pressure rises, they can develop pain, redness, headache, and further visual loss. Slit lamp examination reveals tiny, tangled vessels on the pupillary margin and iris surface, often first seen at the superior pupillary border. Gonioscopy shows neovascularization at the angle and possible early closure. Fluorescein angiography confirms retinal ischemia and helps guide treatment planning.

Management Strategies

Successful management depends on treating the underlying retinal ischemia. Panretinal photocoagulation is used to reduce oxygen demand and lower growth factor production in proliferative retinopathies. Intravitreal injections of anti vascular endothelial growth factor agents can rapidly regress iris and angle neovascularization. Pressure lowering drops and systemic medications help control intraocular pressure, but surgical procedures such as tube shunts are often needed in advanced neovascular glaucoma. Close monitoring is required because recurrence is possible.

Prognosis and Prevention

Prognosis varies with how early rubeosis is detected and how advanced the underlying retinal disease is. Eyes treated in the early neovascular stage before angle closure have a better chance of pressure control and useful vision. Regular retinal screening and timely treatment of diabetic and vascular eye disease are the best ways to prevent rubeosis. In advanced cases, even aggressive therapy may not restore vision, but pain control and eye comfort remain important goals. Protecting the fellow eye through systemic disease control is also critical.
