What Is Microphthalmia?
Microphthalmia is a congenital condition in which one or both eyes are abnormally small due to incomplete development before birth. It exists on a spectrum with anophthalmia, where an eye doesn't form at all. What Does Microphthalmia Look Like?
Parents or clinicians may notice a visibly small eye, deep-set appearance, small cornea, or eyelids that don't fully cover a reduced globe. The condition may affect one eye or both and can occur with coloboma, cataract, or other front-of-eye abnormalities. Some children have normal-looking eyelids but markedly short eye length on measurement.
Orbital and socket growth can lag without adequate volume. Pediatric specialists may use devices called conformers or prosthetic inserts to promote symmetric facial development, even when some vision is present.
What Causes Microphthalmia?
Causes include genetic changes that disrupt eye development, most commonly in genes such as SOX2, OTX2, PAX6, and others. Inheritance patterns vary and can be autosomal dominant, autosomal recessive, or X-linked; many cases occur without a family pattern.
Environmental factors during pregnancy also play a role. Maternal infections, alcohol or certain medication exposure, and other harmful substances have been linked. Isotretinoin is a known cause of severe birth defects and must not be used during pregnancy.
What Are The Symptoms Of Microphthalmia?
Vision depends on the degree of underdevelopment. Some children have usable central vision, while others have profound vision loss, involuntary eye movements, or eye misalignment. Refractive errors and lazy eye are common in single-eye cases and require early attention.
Systemic features may occur in syndromic forms, so pediatric evaluation often includes genetics consultation and, when indicated, brain imaging to look for associated abnormalities. Eye complications can include glaucoma, cataract, and retinal problems that affect the long-term outlook.
Is What Is Microphthalmia Worth Worrying About?
Microphthalmia can limit vision and affect binocular development, school readiness, and mobility. Early diagnosis helps prevent avoidable vision loss from lazy eye, supports safe use of remaining sight, and flags related medical concerns. Families also benefit from understanding recurrence risks and options for future pregnancies.
At the same time, many children do well with coordinated care. Regular follow-up with pediatric eye specialists, genetic counseling, and low vision experts, plus the use of conformers or prosthetics for socket growth when needed, can improve both function and appearance. Community resources and educational accommodations help children participate fully while research advances in genetics and device design continue.
