What Are Iris Transillumination Defects?

Areas of the iris tissue where the posterior pigment epithelium (the layer that blocks light) has been damaged or destroyed, allowing light shined through the pupil to visibly illuminate the iris from behind.

Clinical Appearance

When light is shined into the eye, these defects appear as bright, translucent spots or holes in the iris, visible during a slit lamp examination.

Common Causes

Can be caused by trauma, inflammation (uveitis), certain types of pigment dispersion syndrome (PDS), and sometimes as a side effect of eye surgery.

Pigment Dispersion Syndrome

In PDS, the pigment from the back of the iris flakes off and is deposited elsewhere in the eye, including the drainage angle, potentially leading to Pigmentary Glaucoma.

Is this a sign of injury?

It can be. Defects often appear after blunt force trauma (contusion) to the eye, resulting in tears in the sphincter or dilator muscles of the iris.

How is it diagnosed?

It is diagnosed using a slit lamp microscope by shining a strong, narrow beam of light through the pupil (transillumination) and observing the light leakage through the iris.

Can pigment return?

Usually, no. Once the pigment epithelium is damaged or lost, the defect is permanent, though it may not always affect visual acuity.
