What Is The Difference Between Hemianopia And Anopsia?

Anopsia is a general term for loss of vision or visual field. Hemianopia, also spelled hemianopsia, is a specific type of anopsia in which half of the visual field is missing. When both eyes lose the same side of space, it is called homonymous hemianopsia and usually points to damage behind the optic chiasm.

However, bitemporal hemianopsia is different. It affects the outer halves of both eyes and often indicates compression at the optic chiasm, classically from a pituitary mass. Distinguishing these patterns guides where clinicians look in the brain.

What Causes Hemianopsia?

Stroke is the leading cause in adults, especially infarcts involving the occipital lobe or optic radiations. Hemianopsia can appear suddenly at stroke onset, and some patients recover partially in the first weeks.

Other causes may include:

  • Brain tumors;
  • Head trauma;
  • Injury from brain surgery;
  • Infections or inflammation;
  • Demyelinating disease, and;
  • Less often, seizures or metabolic disorders.

What Are The Symptoms Of Hemianopsia?

People often report running into objects on one side, difficulty finding the start or end of a line when reading, and missing food on one half of the plate. Driving and navigation feel unsafe because pedestrians or cars approaching from the blind side are not detected.

Symptoms may include visual confusion, slow reading, or headaches related to the underlying neurologic problem. Visual acuity can be normal, which is why formal visual field testing is needed even when the eye exam looks healthy.

Is What Is Hemianopsia Worth Worrying About?

Yes, because it signals a brain problem that may need urgent treatment, such as an acute stroke or a mass compressing the optic pathways. It also affects safety, reading, employment, and driving. Early recognition leads to faster stroke workup or tumor evaluation and helps prevent accidents during daily activities.

The good news is that targeted rehabilitation can meaningfully improve independence. Combining medical therapy for the cause with compensatory training, prism lenses, and home strategies such as scanning and anchoring lines of text can reduce disability. A neuro-ophthalmology or vision rehabilitation referral is often helpful.
