What Is Guyon Canal Release?

Guyon canal release is surgery used to decompress the ulnar nerve as it passes through Guyon canal at the wrist. The canal lies on the palm side of the wrist near the pisiform and hook of the hamate. Compression can affect sensation in the little finger and part of the ring finger, hand strength, or both depending on which nerve branch is involved. Surgical release creates more room for the ulnar nerve and its motor and sensory branches.

Why Is Guyon Canal Release Performed?

Surgery is considered when ulnar nerve compression at the wrist causes persistent numbness, weakness, muscle wasting, or functional loss and a structural cause requires decompression. Causes can include ganglion cysts, tumors, hook-of-hamate injuries, vascular abnormalities, scar tissue, or prolonged external pressure. Electrodiagnostic studies and imaging can help distinguish wrist-level compression from cubital tunnel syndrome or another nerve disorder. Mild pressure-related cases can improve without surgery when the underlying cause can be removed or modified.

How Is Guyon Canal Release Performed?

The surgeon makes an incision on the palm side of the wrist and carefully identifies the ulnar nerve before opening the roof of Guyon canal. Constricting fascia and the volar carpal ligament are released while the ulnar artery and nerve branches are protected. The decompression is extended into the appropriate motor or sensory zone when the pattern of compression requires it. A ganglion, scar band, abnormal vessel, or other identified cause can be removed or treated during the same operation.

What Is Recovery Like After Guyon Canal Release?

The wrist and incision are protected during early healing while finger motion is encouraged to reduce stiffness. Heavy gripping, direct pressure on the palm, and forceful wrist activity are restricted until the wound and deeper tissues have recovered. Hand therapy can help restore motion, strength, scar mobility, and nerve-gliding function. Sensory and motor recovery can continue for months, and severe longstanding nerve damage can remain incomplete.

What Are the Risks and Complications?

Possible complications include infection, bleeding, painful scarring, stiffness, persistent numbness, and weakness. Injury to the ulnar nerve, its branches, or the nearby ulnar artery can cause significant hand dysfunction or vascular problems. Symptoms can continue when compression exists at another site or when nerve damage was advanced before surgery. Scar tissue, recurrent masses, or incomplete decompression can lead to recurrent symptoms and occasionally require revision surgery.
