Appearance

These vacuoles scatter light, causing a shimmering or "glistening" appearance within the IOL when viewed under a slit lamp, resembling dewdrops or micro-bubbles.

Clinical Impact

While severe cases are rare, the scattering of light can lead to a slight reduction in visual quality, contrast sensitivity, and an increase in glare or halo effects.

IOL Material

It is most commonly associated with certain types of hydrophobic acrylic IOLs, particularly those with a lower water content, often occurring years after the initial surgery.

Can it be treated?

Most cases are mild and do not require intervention. If vision is significantly impaired, the treatment is IOL exchange (replacement of the lens).

Is this the same as PCO?

No. Posterior Capsule Opacification (PCO) is clouding behind the IOL. Glistening is clouding within the IOL material itself.

Is it preventable?

Newer generations of IOLs made from different materials or manufacturing processes have significantly reduced the incidence of severe glistening.
