Neural Mechanism

Mediated by the neural integrator, a network of brainstem and cerebellar nuclei that convert the brief velocity signal of an eye movement into a sustained tonic (position) signal.

Failure (Nystagmus)

Failure of the gaze-holding mechanism results in the eyes drifting back toward the primary position, followed by a corrective quick phase (jerk nystagmus or gaze-evoked nystagmus).

Clinical Importance

Testing gaze holding is essential for diagnosing disorders of the cerebellum and brainstem, which are often the site of the neural integrator.

What is eccentric gaze?

Eccentric gaze refers to holding the eyes in a position away from the straight-ahead (primary) position, such as looking far left or far right.

Is it related to the vestibular system?

Yes, the neural integrator also helps stabilize the eyes during head movements (Vestibulo-Ocular Reflex), a function related to gaze stability.

What is the difference between Gaze Holding and Fixation?

Fixation is the initial act of pointing the eyes at a target. Gaze Holding is the subsequent act of maintaining that stable position over time.
