Frontal Lobe Lesion

Lesions in the frontal eye fields (e.g., stroke) often cause the eyes to deviate toward the side of the lesion (the eyes "look" at the site of the damage).

Brainstem Lesion

Lesions in the pontine gaze centers (e.g., stroke) typically cause the eyes to deviate away from the side of the lesion (the eyes "look" away from the damage).

Clinical Emergency

Sudden onset of persistent gaze deviation requires immediate medical assessment as it is highly suggestive of a large stroke, seizure, or mass lesion in the brain.

Is this the same as strabismus?

No. Strabismus is an ocular misalignment where the two eyes point in different directions. Gaze deviation involves both eyes pointing in the same wrong direction due to a central nervous system problem.

What is 'forced deviation'?

A severe form of gaze deviation, where the eyes are forcefully and immovably pointed to one side, often seen in large destructive brain lesions.

Can a seizure cause gaze deviation?

Yes. During a seizure, the eyes often deviate away from the side of the active brain focus due to abnormal electrical stimulation.
