Free flap reconstruction is surgery that transfers living tissue from one part of the body to another after completely separating it from its original blood supply. Why Is Free Flap Reconstruction Performed?

Free flaps are used when a wound or surgical defect is too large or complex for direct closure, a skin graft, or a local flap. Common settings include reconstruction after cancer removal, severe trauma, chronic wounds, infection, and complex breast, head and neck, or limb surgery. A flap can bring healthy, well-vascularized tissue into an area with damaged or limited local tissue. The donor site and flap type are selected according to the missing structures, recipient vessels, and functional goals.

How Is Free Flap Reconstruction Performed?

The reconstructive team identifies a suitable donor site and carefully dissects the tissue together with its supplying artery and vein. After the flap is detached, it is moved to the reconstruction site and shaped to fit the defect. Under magnification, the surgeon connects the flap vessels to recipient vessels using very fine sutures or a venous coupling device. The flap is then secured in position, and drains or additional fixation can be placed when required.

What Is Recovery Like After Free Flap Reconstruction?

The flap is monitored closely after surgery for color, temperature, capillary refill, Doppler signals, swelling, and other signs of blood flow. Early vascular blockage can require urgent return to the operating room to restore circulation and attempt to save the flap. Recovery also includes wound care and rehabilitation of both the reconstruction site and donor site. Physical therapy, speech or swallowing therapy, diet changes, or activity restrictions depend on the body area reconstructed.

What Are the Risks and Complications?

Possible complications include bleeding, infection, wound breakdown, blood clots, fluid collections, and problems related to anesthesia. A clot in the microsurgical artery or vein can cause partial or complete flap loss. Donor-site problems can include pain, weakness, numbness, scarring, hernia, or reduced function depending on the tissue removed. Some patients require revision surgery for contour, scar, hardware, or functional problems after healing.

What makes a free flap different from a local flap?

A free flap is completely detached from its original blood supply and reconnected to new vessels. A local or pedicled flap remains attached to its original vascular supply while it is moved.
