What Is Foraminotomy?

Foraminotomy is spinal surgery that enlarges the neural foramen, the opening through which a spinal nerve root exits the spinal canal. The procedure removes bone, ligament, disc material, or other tissue that is narrowing the opening and compressing the nerve. It can be performed in the cervical, thoracic, or lumbar spine, although cervical and lumbar procedures are more common. Foraminotomy is a decompression procedure and does not automatically include spinal fusion.

Why Is Foraminotomy Performed?

Foraminotomy is used to treat nerve-root compression caused by foraminal stenosis, bone spurs, degenerative changes, or selected herniated discs. Symptoms can include radiating arm or leg pain, numbness, tingling, weakness, or reduced function in the area supplied by the affected nerve. Surgery is considered when symptoms are severe, neurological deficits are progressing, or nonsurgical treatment has not provided adequate relief. The goal is to create more space for the nerve root while preserving spinal stability whenever possible.

How Is Foraminotomy Performed?

The procedure can be performed through an open, minimally invasive, microscopic, or endoscopic approach depending on the spinal level and anatomy. The surgeon exposes the affected foramen and removes a controlled amount of bone and soft tissue around the compressed nerve root. Part of the facet joint can be trimmed when it contributes to narrowing. A discectomy can also be performed during the same operation when herniated disc material is compressing the nerve.

What Is Recovery Like After Foraminotomy?

Patients are monitored for pain control, strength, sensation, walking ability, and wound problems after surgery. Many minimally invasive procedures allow early walking and a relatively short hospital stay, but recovery varies by spinal level and extent of decompression. Incisional soreness can persist even when radiating nerve pain improves quickly. Lifting, bending, twisting, driving, and return to work are increased gradually according to symptoms, wound healing, and the surgeon's instructions.

What Are the Risks and Complications?

Possible complications include infection, bleeding, nerve-root injury, numbness, weakness, cerebrospinal fluid leak, persistent pain, and problems related to anesthesia. Removing too much facet joint or supporting bone can contribute to spinal instability or deformity. Recurrent narrowing, scar tissue, or additional disc degeneration can cause symptoms to return later. Some patients eventually require repeat decompression or spinal fusion if instability or recurrent compression develops.

Frequently Asked Questions About Foraminotomy

Is foraminotomy the same as laminectomy?

No. Foraminotomy specifically enlarges the nerve-root exit opening, while laminectomy removes part or all of the lamina to create more space within the spinal canal.

Does foraminotomy always require spinal fusion?

No. Many foraminotomies are performed without fusion. Fusion can be added when instability, deformity, or extensive bone removal makes stabilization necessary.

Can foraminotomy relieve sciatica?

Yes. Lumbar foraminotomy can relieve sciatica when a lumbar nerve root is compressed within or near the neural foramen.

Can symptoms return after foraminotomy?

Yes. Recurrent stenosis, disc degeneration, scar tissue, or new spinal changes can cause nerve compression to develop again over time.
