Enzalutamide is a prescription androgen receptor inhibitor used for several forms of prostate cancer. Current U.S. labeling includes castration-resistant prostate cancer, metastatic castration-sensitive prostate cancer, and nonmetastatic castration-sensitive prostate cancer with biochemical recurrence at high risk for metastasis. It is sold as Xtandi in capsule and tablet forms.

How Does Enzalutamide Affect Androgen Signaling?

Enzalutamide acts at several steps in androgen receptor signaling. It competitively blocks androgen binding to the receptor, reduces movement of the activated receptor into the cell nucleus, and interferes with receptor interaction with DNA. These actions reduce androgen-driven prostate cancer cell growth.

Who Is Treated With Enzalutamide?

Enzalutamide is used for castration-resistant prostate cancer, metastatic castration-sensitive prostate cancer, and high-risk biochemical recurrence of nonmetastatic castration-sensitive prostate cancer. Patients with CRPC or metastatic CSPC also receive a GnRH analog unless they have had bilateral orchiectomy. High-risk biochemical recurrence can be treated with or without a GnRH analog.

How Is Enzalutamide Dosed?

The recommended dose is 160 mg by mouth once daily with or without food. Capsules and tablets are swallowed whole with enough water. Capsules are not opened, dissolved, or chewed, and tablets are not cut, crushed, or chewed. Dose changes are used for selected adverse reactions and drug interactions.

What Serious Risks Are Linked to Enzalutamide?

Serious concerns include seizures, posterior reversible encephalopathy syndrome, hypersensitivity, ischemic heart disease, falls, fractures, severe swallowing difficulty or choking, and fetal harm. Enzalutamide also has substantial drug interaction potential through CYP enzymes. A seizure during treatment calls for permanent discontinuation under current labeling.
