What Is Enoxaparin?

Enoxaparin sodium is a prescription low molecular weight heparin with anticoagulant activity. It is used to prevent deep vein thrombosis in selected surgical and acutely ill medical patients, treat acute deep vein thrombosis, reduce ischemic complications in unstable angina or non-Q-wave myocardial infarction, and treat acute ST-segment elevation myocardial infarction.

How Does Enoxaparin Reduce Blood Clot Formation?

Enoxaparin has antithrombotic activity that is measured largely through inhibition of factor Xa. Its low molecular weight heparin activity reduces formation and extension of fibrin-rich blood clots. The anticoagulant effect is stronger against factor Xa than against thrombin at standard treatment concentrations.

Who Is Treated With Enoxaparin?

Enoxaparin is used for clot prevention after abdominal, hip, or knee surgery and in medical patients with severely restricted mobility during acute illness. It also treats acute deep vein thrombosis with or without pulmonary embolism and is used with aspirin for selected acute coronary syndromes and myocardial infarction.

How Is Enoxaparin Dosed?

Dosing depends on the indication, body weight, age, and kidney function. Hip or knee replacement prophylaxis commonly uses 30 mg subcutaneously every 12 hours. Acute deep vein thrombosis treatment commonly uses 1 mg per kg every 12 hours, while an inpatient regimen of 1.5 mg per kg once daily is another labeled option.

What Serious Risks Are Linked to Enoxaparin?

Enoxaparin carries a boxed warning for spinal or epidural hematomas in patients receiving neuraxial anesthesia or spinal puncture. Other serious concerns include major bleeding, heparin-induced thrombocytopenia with or without thrombosis, low platelet counts, and greater drug exposure in severe kidney impairment. Enoxaparin is not interchangeable unit for unit with other heparins.
