How Does Endari Act in Sickle Cell Disease?

The mechanism of L-glutamine in sickle cell disease is not fully defined. Oxidative stress contributes to sickle red blood cell damage. In vivo analyses described in the labeling showed improved NAD redox potential after L-glutamine supplementation. This redox pathway is linked to protection against oxidative damage in red blood cells.

Who Is Treated With Endari?

Endari is used in adults and children age 5 and older with sickle cell disease to reduce acute disease complications. Safety and efficacy have not been established in children younger than 5. The medicine is taken orally twice daily using a body-weight-based dose.

How Is Endari Dosed?

Patients weighing less than 30 kg take 5 g twice daily. Those weighing 30 kg to 65 kg take 10 g twice daily, and those above 65 kg take 15 g twice daily. Each dose is mixed with 8 oz of a cold or room-temperature beverage or 4 oz to 6 oz of food.

Current labeling lists constipation, nausea, headache, abdominal pain, cough, pain in an extremity, back pain, and chest pain among the most common adverse reactions.
