How Does Elagolix Affect Ovarian Hormones?

Elagolix competitively binds gonadotropin-releasing hormone receptors in the pituitary gland and reduces GnRH signaling. This lowers luteinizing hormone and follicle-stimulating hormone secretion, followed by dose-dependent reductions in estradiol and progesterone concentrations.

Who Is Treated With Elagolix?

Elagolix is used for moderate to severe pain associated with endometriosis. Pregnancy is excluded before treatment unless therapy begins within seven days from the onset of menses. It is contraindicated during pregnancy, in known osteoporosis, in severe liver impairment, and with certain OATP1B1 inhibitors.

How Is Elagolix Dosed?

With normal liver function or mild liver impairment, 150 mg once daily can continue for up to 24 months, while 200 mg twice daily is limited to six months. Moderate liver impairment uses 150 mg once daily for up to six months. Tablets are taken at about the same time each day with or without food.

What Serious Risks Are Linked to Elagolix?

Elagolix causes dose-dependent bone mineral density loss that can be only partly reversible after treatment ends. Other serious concerns include reduced recognition of pregnancy due menstrual changes, suicidal thoughts or behavior, worsening mood disorders, liver enzyme elevations, and interactions with hormonal contraceptives. Nonhormonal contraception is used during treatment and for 28 days afterward.
