Purpose of the Reflex

Its primary function is to regulate the amount of light reaching the retina, protecting the photoreceptors from damage in bright conditions and improving visual depth of field.

Neural Pathway

Involves the optic nerve (afferent path) sensing the light and the oculomotor nerve (efferent path) signaling the iris sphincter muscle to contract.

Comparison to Consensual Reflex

The direct reflex occurs in the illuminated eye, while the consensual light reflex is the simultaneous constriction that occurs in the opposite, non-illuminated eye.

Is the reflex always equal in both eyes?

The reflexes should be equal (direct and consensual) in healthy eyes. An unequal response (relative afferent pupillary defect) can signal optic nerve damage.

What muscle causes the constriction?

The sphincter pupillae muscle, controlled by the parasympathetic nervous system, causes the pupil to constrict (miosis).

How is the reflex tested clinically?

A doctor shines a penlight into one eye and observes the reaction of both the illuminated eye (direct) and the opposite eye (consensual).
