How Does Diacomit Affect Seizures?

The exact anticonvulsant mechanism of stiripentol in humans is unknown. Proposed actions include direct effects involving GABA-A receptors and indirect effects from inhibition of cytochrome P450 enzymes, which increases blood concentrations of clobazam and its active metabolite.

Who Is Treated With Diacomit?

Diacomit treats seizures associated with Dravet syndrome in patients taking clobazam who are at least 6 months old and weigh at least 7 kg. Current labeling states that clinical data do not establish Diacomit as monotherapy for Dravet syndrome.

How Is Diacomit Dosed?

The recommended total dose is 50 mg per kg daily, divided into two or three oral doses according to age and body weight. The maximum is 3000 mg daily. Capsules are swallowed whole with water during a meal. Powder is mixed with water and taken immediately during a meal.

What Serious Risks Are Linked to Diacomit?

Diacomit can cause sleepiness, decreased appetite, weight loss, neutropenia, thrombocytopenia, and suicidal thoughts or behavior. Blood counts are checked before treatment and every six months. Abrupt withdrawal can increase seizure frequency or trigger status epilepticus. The powder contains phenylalanine, which matters for patients with phenylketonuria.
