What Is Clindamycin?

Clindamycin is a prescription lincosamide antibacterial medicine used for selected serious infections caused by susceptible bacteria. Oral clindamycin hydrochloride capsules are used for serious infections involving anaerobic bacteria and susceptible streptococci, pneumococci, or staphylococci. Other clindamycin formulations have separate topical, vaginal, and injectable indications.

How Does Clindamycin Stop Bacterial Growth?

Clindamycin binds to the 23S RNA of the bacterial 50S ribosomal subunit and inhibits protein synthesis. Its usual action is bacteriostatic, which means it stops susceptible bacteria from multiplying. Resistance can occur through changes to ribosomal binding sites, and cross-resistance can occur with lincomycin and selected other antibacterial classes.

Which Infections Are Treated With Oral Clindamycin?

Oral clindamycin treats selected serious respiratory, skin and soft tissue, intra-abdominal, pelvic, bloodstream, and other infections caused by susceptible organisms. Current labeling reserves oral clindamycin for serious infections when less toxic antibacterial medicines are inappropriate, including selected patients who cannot use penicillin.

How Are Clindamycin Capsules Dosed?

Adults with serious infections commonly take 150 mg to 300 mg every six hours. More severe infections use 300 mg to 450 mg every six hours. Pediatric capsule dosing is based on body weight and infection severity. Capsules are swallowed with a full glass of water to reduce the risk of esophageal irritation.

What Serious Risks Are Linked to Clindamycin?

Clindamycin carries a boxed warning for Clostridioides difficile-associated diarrhea and severe colitis, which can occur during treatment or after the antibiotic has been stopped. Other serious reactions include severe allergic reactions, serious skin reactions, liver abnormalities, low blood cell counts, and esophageal irritation.
