Cefpodoxime is an oral cephalosporin antibacterial given as the prodrug cefpodoxime proxetil. Tablets come in 100 mg and 200 mg strengths, and an oral suspension is also available. It treats selected mild to moderate bacterial infections of the ear, throat, lungs, sinuses, skin, and urinary tract, along with selected uncomplicated gonococcal infections.

What Is Cefpodoxime?

How Does Cefpodoxime Stop Bacterial Growth?

Cefpodoxime is bactericidal and inhibits bacterial cell wall synthesis. After cefpodoxime proxetil is absorbed, it is converted to active cefpodoxime. The active drug binds bacterial penicillin-binding proteins and disrupts peptidoglycan formation needed for cell wall integrity. Resistance can develop through beta-lactamases, altered penicillin-binding proteins, or reduced bacterial permeability.

Which Infections Are Treated With Cefpodoxime?

Cefpodoxime is labeled for selected cases of acute otitis media, pharyngitis or tonsillitis, community-acquired pneumonia, bacterial exacerbations of chronic bronchitis, acute maxillary sinusitis, uncomplicated skin infections, uncomplicated urinary tract infections, and uncomplicated gonorrhea caused by susceptible bacteria. Age limits and approved dosage forms differ across these infections.

How Is Cefpodoxime Dosed?

Adults and adolescents age 12 and older commonly receive 100 mg or 200 mg every 12 hours for several respiratory or urinary infections, with higher dosing for skin infections. Tablets are taken with food to improve absorption. Pediatric oral suspension dosing is weight based. Severe kidney impairment lengthens the dosing interval.

What Safety Problems Need Attention With Cefpodoxime?

Serious hypersensitivity reactions can occur, especially in people with prior beta-lactam allergy. Clostridioides difficile-associated diarrhea is another concern and can appear during or after antibacterial treatment. Diarrhea, nausea, abdominal pain, and vaginal fungal infections are reported reactions. Cefpodoxime is contraindicated in people with a known allergy to cefpodoxime or cephalosporins.
