Cefdinir is a prescription cephalosporin antibacterial used for mild to moderate infections caused by susceptible bacteria. Adult and adolescent capsule indications include community-acquired pneumonia, bronchitis exacerbations, acute maxillary sinusitis, pharyngitis or tonsillitis, and uncomplicated skin infections. Pediatric formulations also have labeled uses for selected ear, throat, and skin infections.

How Does Cefdinir Stop Bacterial Growth?

Cefdinir kills susceptible bacteria by inhibiting bacterial cell wall synthesis. It remains stable in the presence of some beta-lactamase enzymes, though other resistance mechanisms can reduce activity. Labeled susceptible organisms include selected strains of Streptococcus pneumoniae, Streptococcus pyogenes, Staphylococcus aureus, Haemophilus influenzae, Haemophilus parainfluenzae, and Moraxella catarrhalis.

Which Infections Are Treated With Cefdinir?

In adults and adolescents, cefdinir capsules treat selected community-acquired pneumonia, acute exacerbations of chronic bronchitis, acute maxillary sinusitis, streptococcal pharyngitis or tonsillitis, and uncomplicated skin and skin structure infections. Pediatric labeling also includes acute bacterial otitis media and selected throat and skin infections. Use depends on bacterial susceptibility.

How Are Cefdinir Capsules Dosed?

Adults and adolescents age 13 and older receive a total of 600 mg daily. Depending on the infection, this is given as 300 mg every 12 hours or 600 mg once daily. Pneumonia and uncomplicated skin infections use 300 mg every 12 hours. Marked kidney impairment requires a lower daily dose.

What Safety Problems Need Attention With Cefdinir?

Serious hypersensitivity reactions, including anaphylaxis, can occur with cefdinir. Clostridioides difficile-associated diarrhea can develop during or after treatment. Other reported reactions include severe skin disorders, kidney dysfunction, liver dysfunction, blood cell abnormalities, and seizures, particularly with excessive exposure during kidney impairment. Cefdinir is contraindicated in people with known cephalosporin allergy.
