How Do Carbidopa and Levodopa Work Together?

Levodopa crosses the blood-brain barrier and is converted to dopamine in the brain, helping replace deficient dopamine activity. Carbidopa does not cross the blood-brain barrier to a substantial degree and inhibits peripheral levodopa decarboxylation. This sends more levodopa to the brain and reduces peripheral reactions such as nausea and vomiting.

Who Is Treated With Carbidopa and Levodopa?

Carbidopa and levodopa tablets are used for Parkinson disease and parkinsonian syndromes. The combination can treat parkinsonism that follows encephalitis or carbon monoxide or manganese intoxication. Dosing is individualized according to symptoms, prior levodopa exposure, response, and adverse reactions.

How Are Carbidopa and Levodopa Tablets Dosed?

Immediate-release treatment commonly starts with one 25 mg with 100 mg tablet three times daily. The dose can increase by one tablet every day or every other day according to response, up to eight 25 mg with 100 mg tablets daily under the usual schedule. Vitamin B6 levels are evaluated before and during treatment.

What Safety Problems Need Attention With Carbidopa and Levodopa?

Carbidopa and levodopa can cause sudden sleep episodes, sleepiness, dyskinesia, hallucinations, psychotic-like behavior, compulsive behaviors, and low blood pressure. Rapid dose reduction or withdrawal can trigger high fever, muscle rigidity, confusion, and autonomic instability. Current labeling also warns about vitamin B6 deficiency and associated seizures during treatment.
