How Do Cabotegravir and Rilpivirine Block HIV-1?

Cabotegravir is an integrase strand transfer inhibitor that blocks insertion of viral DNA into human cell DNA. Rilpivirine is a non-nucleoside reverse transcriptase inhibitor that binds HIV-1 reverse transcriptase and disrupts viral DNA synthesis. Together, these long-acting antiretroviral medicines maintain suppression of HIV-1 replication between scheduled injection visits.

Who Is Treated With Cabenuva?

Cabenuva is used in adults and adolescents age 12 and older who weigh at least 35 kg and already have HIV-1 RNA below 50 copies per mL on a stable regimen. Eligible patients have no history of treatment failure and no known or suspected resistance to cabotegravir or rilpivirine.

How Are Cabenuva Injections Scheduled?

Cabenuva is given by a healthcare professional as two separate gluteal intramuscular injections during the same visit. Treatment can follow a monthly or every-two-month schedule. An oral lead-in with cabotegravir 30 mg and rilpivirine 25 mg daily for at least 28 days is optional before injections. Direct initiation with injections is also permitted.

What Safety Problems Need Attention With Cabenuva?

Serious risks include hypersensitivity reactions, post-injection reactions, liver toxicity, depressive disorders, and loss of virologic response with drug interactions or missed doses. Long-acting drug concentrations can remain in the body for 12 months or longer after the last injections. Scheduled visits and timely replacement antiretroviral treatment after discontinuation reduce the risk of resistance.
