How Do the Medicines in Breyna Act in the Airways?

Budesonide binds glucocorticoid receptors and reduces inflammatory activity in the airways. Formoterol stimulates beta2-adrenergic receptors on airway smooth muscle, increasing cyclic AMP and relaxing bronchial muscle. The combination pairs anti-inflammatory corticosteroid activity with long-acting bronchodilation for ongoing asthma or chronic obstructive pulmonary disease treatment.

Who Is Treated With Breyna?

Breyna is approved for asthma in patients age 6 and older. It is also approved for maintenance treatment of airflow obstruction and reduction of exacerbations in chronic obstructive pulmonary disease, including chronic bronchitis or emphysema. It is not indicated for relief of sudden bronchospasm.

How Is Breyna Dosed?

Patients age 12 and older with asthma use two inhalations of 80 mcg with 4.5 mcg or 160 mcg with 4.5 mcg twice daily. Children age 6 to under 12 use two inhalations of 80 mcg with 4.5 mcg twice daily. Chronic obstructive pulmonary disease uses two inhalations of 160 mcg with 4.5 mcg twice daily.

What Safety Problems Need Attention With Breyna?

Breyna can cause oral candidiasis, lower respiratory infections, adrenal effects, reduced bone mineral density, slowed growth in children, cataracts, glaucoma, paradoxical bronchospasm, and cardiovascular effects. Patients rinse the mouth with water without swallowing after inhalation. A short-acting bronchodilator is used for sudden breathing symptoms.
