What Causes Bitemporal Hemianopia?

This condition is most often caused by a problem with the optic chiasm. The optic chiasm is where parts of the optic nerves from each eye cross in the brain. The most common cause is a tumor on the pituitary gland, which sits just below the optic chiasm. As the tumor grows, it can press on the nerve fibers and damage them.

The main symptom is a slow loss of your peripheral, or side, vision. People with this condition often bump into objects on either side of them. It can make activities like driving very difficult and unsafe. Because the vision loss happens gradually, some people may not notice it at first.

How Is Bitemporal Hemianopia Treated?

Treatment focuses on the underlying cause of the problem. If a pituitary tumor is causing the vision loss, treatment may include surgery to remove it, radiation, or medication. The goal is to take the pressure off the optic chiasm. Sometimes, vision can improve after treatment, but if the nerves were damaged for a long time, the vision loss may be permanent.

Visualizing the Optic Chiasm

To understand why this specific vision loss happens, you have to look at the brain's wiring. The optic nerves from both eyes meet at an X-shaped junction called the optic chiasm. Here, the nerve fibers responsible for your peripheral (side) vision cross over to the opposite side of the brain. Because the pituitary gland sits directly underneath this "X," a tumor growing upward pushes right into the center of the cross. This compresses only the crossing fibers, effectively turning off the side vision for both eyes while leaving central vision intact.

Hormonal Warning Signs

Since the most common cause is a pituitary tumor, your eyes might not be the only warning sign. The pituitary gland is the "master gland" of the body. As the tumor presses on the gland, it can disrupt hormone production. Patients often experience systemic symptoms alongside vision loss, such as unexplained weight gain or loss, fatigue, temperature sensitivity, or changes in libido.

Can bitemporal hemianopia be cured?

If the cause is treated early, some or all of the vision may return. However, if the pressure on the optic nerve lasts for too long, the vision loss can be permanent. Early diagnosis makes a big difference.

Does it affect both eyes the same way?

It affects the outer vision in both eyes, but the amount of vision loss might not be exactly the same in each eye. One side might be worse than the other.

Is it safe to drive with this condition?

No, it is generally not safe or legal to drive with bitemporal hemianopia. The loss of side vision creates dangerous blind spots that prevent you from seeing other cars or people.

When to See Your Eye Doctor

You should see a doctor if you notice any changes in your side vision. If you find yourself frequently bumping into things or feel that your field of view is getting smaller, schedule an eye exam. An eye doctor can perform tests to check your visual field and find the cause of the problem.
