How Do the Medicines in Biktarvy Block HIV-1?

Bictegravir blocks HIV-1 integrase and prevents viral DNA from being inserted into human cell DNA. Emtricitabine and tenofovir alafenamide form active metabolites that inhibit HIV-1 reverse transcriptase and stop viral DNA chain growth. Acting at different stages of replication suppresses production of new HIV-1 particles.

Who Is Treated With Biktarvy?

Biktarvy is approved for adults and children weighing at least 14 kg with HIV-1. The current label covers people with no prior antiretroviral treatment, selected people who previously received antiretroviral therapy and are not virologically suppressed without specified resistance substitutions, and virologically suppressed patients switching from a stable regimen when resistance criteria are met.

How Is Biktarvy Dosed?

Adults and children weighing at least 25 kg take one tablet containing 50 mg bictegravir, 200 mg emtricitabine, and 25 mg tenofovir alafenamide once daily. Children weighing 14 kg to under 25 kg take the lower-strength tablet once daily. Biktarvy is taken with or without food.

What Major Risks Come With Biktarvy?

Biktarvy carries a boxed warning for severe acute hepatitis B exacerbation after treatment stops in people with HIV-1 and hepatitis B coinfection. Other concerns include kidney impairment, lactic acidosis, severe liver enlargement with fat accumulation, immune reconstitution syndrome, and clinically significant drug interactions. Dofetilide and rifampin are contraindicated.
