How Does Berinert Control Hereditary Angioedema Swelling?

C1 esterase inhibitor is a normal plasma protein that regulates several enzyme cascade systems, including complement and contact pathways. In hereditary angioedema, deficient or dysfunctional C1 inhibitor permits excessive generation of mediators linked to swelling. Berinert raises functional C1 inhibitor activity and restores regulation of these pathways during an acute attack.

When Is Berinert Used?

Berinert is approved for acute abdominal, facial, or laryngeal hereditary angioedema attacks in adult and pediatric patients. Its prescribing information does not establish prophylactic use. Laryngeal attacks can obstruct the airway, so patients who self-administer Berinert for throat swelling are instructed to seek immediate medical care after treatment.

How Is Berinert Given?

The labeled dose is 20 International Units per kg of body weight by intravenous injection. The freeze-dried powder is reconstituted with the supplied sterile water before use. Patients or caregivers can self-administer after proper training. Doses lower than 20 International Units per kg are not recommended under the prescribing information.

What Serious Risks Are Linked to Berinert?

Berinert can cause severe hypersensitivity reactions, including hives, chest tightness, wheezing, low blood pressure, and anaphylaxis. Arterial and venous thromboembolic events have also been reported with C1 esterase inhibitor products. Since Berinert is made from human plasma, infectious agent transmission remains a theoretical risk despite manufacturing steps that reduce that risk.
