Azulfidine is a prescription medicine containing sulfasalazine, an anti-inflammatory drug. The standard 500 mg tablet is approved for mild to moderate ulcerative colitis, as adjunctive therapy in severe ulcerative colitis, and for prolonging remission between acute attacks. A delayed-release product called Azulfidine EN-tabs has separate indications that also include rheumatoid arthritis and polyarticular-course juvenile rheumatoid arthritis.

How Does Azulfidine Act in Ulcerative Colitis?

The exact mechanism of sulfasalazine is not fully defined. Intestinal bacteria split sulfasalazine into sulfapyridine and 5-aminosalicylic acid. Clinical studies point to 5-aminosalicylic acid as the main contributor to therapeutic activity in ulcerative colitis. Its local anti-inflammatory activity in intestinal tissue is thought to account for much of the response.

When Is Azulfidine Used?

Standard Azulfidine tablets are approved for mild to moderate ulcerative colitis, as adjunctive therapy in severe ulcerative colitis, and for prolonging remission between acute attacks. Azulfidine EN-tabs has these ulcerative colitis indications plus rheumatoid arthritis and polyarticular-course juvenile rheumatoid arthritis indications under labeled criteria. The two tablet forms have different labeling and should not be treated as identical products.

How Are Azulfidine Tablets Dosed?

Adults with active ulcerative colitis commonly receive 3 to 4 g daily in evenly divided doses, with intervals no longer than eight hours. Adult maintenance treatment uses 2 g daily. Children age 6 and older receive weight-based dosing. Initial pediatric dosing is 40 to 60 mg per kg daily, followed by 30 mg per kg daily for maintenance.

Which Reactions Need Medical Attention With Azulfidine?

Serious risks include agranulocytosis, aplastic anemia, other blood disorders, liver injury, kidney injury, serious hypersensitivity, and severe infections. Fever, sore throat, pallor, purpura, jaundice, rash, or declining kidney function call for prompt medical review. Complete blood counts, liver tests, urinalysis, and renal function are checked during treatment to detect toxicity early.
