What Causes Aphakia?

Aphakia happens when the eye is missing its natural lens. The lens sits behind the colored part of the eye (the iris) and helps focus light, so when it is gone, the eye loses a big part of its focusing power.

Most cases of aphakia are acquired, meaning they happen later in life. The most common cause is cataract surgery, where the cloudy natural lens is removed. Today, many people receive an artificial lens implant (an intraocular lens) during the same procedure, but aphakia can occur if an implant is not placed or cannot be safely placed.

Aphakia can also happen after eye injuries. A hard hit or blunt trauma can tear the tiny fibers that hold the lens in place, causing the lens to shift (dislocate) or be damaged so severely that it has to be removed. Less commonly, aphakia is present at birth (congenital aphakia) due to rare developmental conditions affecting the lens during pregnancy.

Common causes include:

  • Cataract surgery without an intraocular lens implant

  • Complicated cataract surgery where an implant cannot be placed right away

  • Blunt trauma that dislocates or damages the lens

  • Rare congenital (from birth) lens development problems

What Complications Can Aphakia Cause?

If left untreated, aphakia can make everyday tasks like reading or driving difficult because of constant blurry or distorted vision. It can also increase sensitivity to light and raise the risk of other eye problems, such as retinal detachment or glaucoma.

Regular follow-ups with an eye doctor are important to manage these risks and maintain clear, healthy vision.

Amblyopia Risk in Children (When Vision Stays Blurry Too Long)

In pediatric aphakia, clear focus is not just about seeing better. It shapes how the visual system develops. If one eye stays out of focus for too long, the brain starts relying on the clearer eye and tunes out the blurrier one. Over time, that weaker eye can fall behind, leading to amblyopia (lazy eye). Strabismus (eye misalignment) can also happen when the eyes stop working as a team.

Early correction (with contacts, glasses, or a surgeon-recommended lens option) plus regular checkups helps keep both eyes developing normally while a child’s vision is still forming.

What Are the Warning Signs of Aphakia?

If you have aphakia, vision often becomes very blurry because the eye is missing its natural lens, which normally focuses light onto the retina. Many people notice blur at both distance and near, and near work is often the most difficult because the eye can no longer shift focus up close.

You might also feel eye strain or notice double vision, especially if only one eye is aphakic. When one eye sees a different image size or clarity than the other, the eyes may not work together smoothly.

Light sensitivity is also common. Bright sunlight, screens, and car headlights can trigger glare or halos, and some people notice lower contrast or more trouble seeing in dim light.


Common symptoms include:

  • Blurry vision at distance and near (near is often worse)
  • Trouble reading or focusing up close
  • Eye strain or double vision, especially with one-eye aphakia
  • Glare, halos, or a washed-out look in bright light
  • Reduced contrast or night-vision difficulty

During an eye exam, aphakia often shows up as a very strong farsighted (high plus) prescription, since the eye has lost a large part of its focusing power. Your eye doctor may also check for related findings, such as changes in the pupil, signs of prior surgery or trauma, and whether the front of the eye is stable. Correction with contact lenses, glasses, or a lens implant is usually needed to restore clear focus and reduce symptoms like blur and glare.

When Should You See an Eye Doctor?

You should see an eye doctor if you've had cataract surgery and notice blurry vision, double vision, or difficulty focusing. Aphakia, which means the absence of the eye's natural lens, can cause significant refractive errors and light sensitivity. Regular checkups are important to monitor your vision and fit the correct corrective lenses or intraocular implants.

Regular eye exams are one of the best ways to detect and manage eye diseases early. Many eye conditions develop gradually and may not show noticeable symptoms until they start affecting vision. Visiting an eye doctor routinely helps protect your eyesight and maintain good eye health over time.

Can Aphakia Be Cured?

The missing natural lens can't grow back, but vision can be restored. Eye surgeons often replace it with an artificial intraocular lens or commonly known as IOL. You can also use special contact lenses or glasses to correct your focus. Most people with Aphakia regain clear sight with the right correction.

How to Prevent Aphakia?

Not every cause of aphakia is avoidable, but you can lower your risk from eye injuries. Wear protective eyewear during sports and in hazardous work environments and treat eye infections or inflammation quickly to prevent damage. When cataracts appear, ask your eye doctor about lens-sparing surgery options.

Is Aphakia Inherited?

Most cases follow surgery or injury, not family traits.

Can I See Normally With Aphakia?

Yes. An IOL implant or strong corrective lenses can restore clear vision.
