What Are Oxytocic Agents Used For?

Oxytocin is used to induce labor for appropriate medical indications and to strengthen inadequate contractions during labor. Uterotonic medicines are also used to prevent or treat postpartum hemorrhage caused by poor uterine tone. Methylergonovine, carboprost, and misoprostol are additional options in selected postpartum settings. Product selection depends on urgency, bleeding severity, blood pressure, asthma, prior uterine surgery, and other maternal factors.

How Do Oxytocic Agents Work?

Oxytocin activates oxytocin receptors and increases intracellular calcium in uterine muscle cells. Ergot derivatives produce sustained uterine contraction through serotonergic, adrenergic, and other receptor activity. Prostaglandins stimulate uterine contraction and can also soften or ripen the cervix. The pattern of contraction can range from rhythmic activity to prolonged tone depending on the drug and dose.

How Are Oxytocic Agents Given?

Oxytocin is commonly given as a carefully controlled intravenous infusion for labor induction or augmentation and can be administered after delivery by approved routes. Other uterotonics can be given orally, buccally, sublingually, rectally, intramuscularly, or by another product-specific route. Maternal contractions, fetal heart rate, blood pressure, fluid balance, and bleeding require monitoring. Administration for labor must occur where immediate obstetric and emergency support is available.

What Are the Side Effects and Precautions?

Excessive uterine stimulation can reduce fetal oxygen delivery, cause abnormal fetal heart patterns, or contribute to uterine rupture. Oxytocin can cause water intoxication and low sodium when high doses or large volumes are used for prolonged periods. Ergot agents can cause severe hypertension or vascular spasm, while carboprost can trigger bronchospasm. The risks require careful product selection, dose control, and continuous assessment.
