What Are Ocular Inserts Used For?

Ocular inserts can be used for dry-eye symptoms, inflammation, pain after eye surgery, glaucoma treatment, or other product-specific conditions. Hydroxypropyl cellulose inserts provide prolonged lubrication for selected patients with dry eye. Dexamethasone intracanalicular inserts release corticosteroid after ophthalmic procedures. Intraocular implants are related sustained-delivery systems but are placed inside the eye and are not handled like conjunctival inserts.

How Do Ocular Inserts Work?

Tear fluid can hydrate or dissolve an insert and release its contents gradually. Other products use biodegradable polymers, diffusion-controlled matrices, or channels that regulate drug movement. Prolonged contact can extend local exposure and reduce the need for frequent drops. Release rate depends on the material, drug properties, placement site, tear environment, and product design.

How Is an Ocular Insert Placed?

Placement depends on the product and can involve the conjunctival sac, tear drainage canal, or another ocular location. Some inserts are placed by the patient with an applicator after training, while others require an eye-care professional. Hands, applicators, and packaging must be handled according to sterile or clean-use instructions. The insert should not be moved, reused, or removed unless the product directions or clinician call for it.

What Are the Side Effects and Precautions?

Possible effects include foreign-body sensation, blurred vision, tearing, redness, irritation, swelling, or accidental displacement. Drug-releasing inserts can also cause the adverse effects associated with the active medicine, such as increased eye pressure with corticosteroids. Incorrect placement can scratch the ocular surface or reduce treatment effectiveness. Eye pain, marked redness, discharge, sudden vision change, or a missing insert requires prompt professional advice.
