What Are Azole Antifungals Used For?

Azole antifungals treat infections ranging from superficial skin or vaginal yeast infections to serious systemic disease. Depending on the drug, they can be used for candidiasis, dermatophyte infections, aspergillosis, cryptococcosis, histoplasmosis, and other mycoses. Some are also given to prevent fungal infection in patients at high risk. Each azole has a different spectrum, tissue distribution, and approved use.

How Do Azole Antifungals Work?

Azoles inhibit a fungal cytochrome P450 enzyme involved in converting lanosterol into ergosterol. Reduced ergosterol weakens the fungal cell membrane and disrupts its function. The resulting effect can stop fungal growth or kill susceptible fungi, depending on the organism and drug concentration. Resistance can develop through changes in the target enzyme, increased target production, or drug-removing pumps.

How Are Azole Antifungals Given?

Azole antifungals are available as creams, powders, vaginal products, oral medicines, eye preparations, and intravenous formulations. The route and duration depend on the infection site, organism, severity, and immune status. Absorption of some oral azoles is affected by food, stomach acidity, or formulation. Patients should follow product-specific directions because dosing instructions are not the same across the class.

What Are the Side Effects and Drug Interactions?

Common effects can include nausea, abdominal discomfort, diarrhea, headache, or local irritation. Systemic azoles can cause liver injury, and some can affect the QT interval or other aspects of heart rhythm. Many azoles inhibit or use liver enzymes that process other medicines, creating potentially serious drug interactions. Pregnancy, liver disease, heart conditions, and all current medicines should be reviewed before systemic treatment.

Are azole antifungals antibiotics?
