What Are Aminosalicylates Used For?

How Do Aminosalicylates Work?

The exact mechanism is not fully defined, but 5-aminosalicylic acid reduces several inflammatory pathways within the intestinal lining. It affects prostaglandins, leukotrienes, cytokines, nuclear signaling, and activity of inflammatory cells. The effect is largely topical, so delivery of active medicine to the inflamed bowel segment is important. Different coatings, carriers, and prodrugs control where release occurs.

How Are Aminosalicylates Taken?

Products include immediate-release, delayed-release, extended-release, and multimatrix oral formulations as well as rectal suppositories and enemas. Tablets or capsules should be swallowed, opened, or divided only according to their specific instructions. Sulfasalazine is converted by colonic bacteria into 5-aminosalicylic acid and sulfapyridine. Adherence can be challenging because some regimens involve several tablets or both oral and rectal therapy.

What Are the Side Effects and Monitoring Needs?

Possible effects include headache, nausea, abdominal pain, diarrhea, rash, and worsening colitis-like symptoms from acute intolerance. Rare risks include kidney injury, pancreatitis, liver injury, blood disorders, myocarditis, pericarditis, or severe skin reactions. Sulfasalazine can cause additional effects related to its sulfapyridine component, including reversible reduction in sperm count and impaired folate absorption. Kidney function is evaluated before treatment and periodically during therapy.
