What Are Tumor Necrosis Factor Inhibitors Used For?

TNF inhibitors treat selected inflammatory diseases such as rheumatoid arthritis, psoriatic arthritis, axial spondyloarthritis, plaque psoriasis, and juvenile idiopathic arthritis. Depending on the product, uses also include Crohn disease, ulcerative colitis, hidradenitis suppurativa, and noninfectious uveitis. Approved indications differ because the drugs are not identical. Treatment is commonly considered when disease activity warrants systemic immune-modifying therapy.

How Do Tumor Necrosis Factor Inhibitors Work?

Tumor necrosis factor promotes immune-cell activation, inflammatory signaling, blood-vessel changes, and tissue damage. Monoclonal antibodies bind soluble and membrane-associated tumor necrosis factor to reduce receptor activation. Etanercept is a fusion protein that acts as a soluble decoy receptor. Blocking this pathway can reduce inflammation but can also impair normal responses to infection and malignancy.

How Are Tumor Necrosis Factor Inhibitors Given?

Adalimumab, certolizumab, golimumab, and etanercept have subcutaneous formulations, while infliximab is administered by intravenous infusion. Dosing intervals range from weekly injections to infusions separated by several weeks. Patients are screened for tuberculosis and commonly hepatitis B before treatment. Vaccinations, infection history, heart failure, neurological disease, cancer history, and other immunosuppressive medicines are reviewed.

What Are the Side Effects and Precautions?

TNF inhibitors increase the risk of serious bacterial, fungal, viral, and opportunistic infections, including tuberculosis reactivation. Other risks include injection or infusion reactions, hepatitis B reactivation, blood-cell abnormalities, demyelinating disease, worsening heart failure, lupus-like reactions, and selected malignancies. Live vaccines are generally avoided during treatment. Fever, persistent cough, shortness of breath, neurological symptoms, or signs of severe infection require prompt assessment.
