A somatostatin analog is a synthetic medicine designed to imitate actions of the natural hormone somatostatin. These drugs bind to one or more somatostatin receptor subtypes and suppress release of selected hormones and signaling molecules. What Are Somatostatin Analogs Used For?

Somatostatin analogs are used to treat acromegaly and selected hormone-producing neuroendocrine tumors. Octreotide and lanreotide can reduce flushing and diarrhea associated with carcinoid syndrome and can control symptoms from certain other functional tumors. Some formulations can slow progression of selected gastroenteropancreatic neuroendocrine tumors. Pasireotide has approved uses for selected pituitary disorders, including Cushing disease or acromegaly, depending on the formulation.

How Do Somatostatin Analogs Work?

Somatostatin receptors are G protein-coupled receptors found in the pituitary gland, gastrointestinal tract, pancreas, and many neuroendocrine tumors. Activating these receptors reduces secretion of growth hormone and several gastrointestinal or pancreatic hormones. The drugs can also reduce blood flow and secretory activity within the digestive system. Clinical effects differ because each analog has a distinct affinity for the five somatostatin receptor subtypes.

How Are Somatostatin Analogs Given?

Products include short-acting subcutaneous injections, long-acting intramuscular injections, deep subcutaneous depot injections, and selected oral capsules. Long-acting formulations are administered at intervals that can extend for several weeks. Dosing is adjusted according to symptoms, hormone levels, tumor response, adverse effects, and product-specific instructions. Some patients receive a short-acting formulation before transition to a long-acting product.

What Are the Side Effects and Monitoring Needs?

Common effects include abdominal discomfort, nausea, diarrhea, constipation, injection-site reactions, and changes in blood glucose. Reduced gallbladder contraction can lead to gallstones or biliary complications during long-term treatment. Bradycardia, thyroid changes, vitamin B12 changes, liver-test abnormalities, and cardiac-conduction effects can occur with selected products. Monitoring can include glucose, gallbladder symptoms, thyroid function, heart rate, liver tests, hormone levels, and tumor assessment.
