What Is a Fetal Scalp Electrode Used For?

A fetal scalp electrode is used when more accurate or continuous fetal heart rate monitoring is needed during labor. It may be considered when external monitoring is difficult because of movement, body position, body size, or unclear tracing quality. The electrode helps the care team assess fetal heart rate patterns while labor continues. It does not measure contractions or oxygen levels by itself.

How a Fetal Scalp Electrode Works

The electrode detects electrical signals from the fetal heart through contact with the fetal skin. Those signals are sent through a wire to the fetal monitoring system. Because the sensor is attached directly to the fetus, the tracing can be more consistent than external Doppler monitoring. The monitor displays fetal heart rate patterns for the care team to interpret.

How Is a Fetal Scalp Electrode Placed?

The clinician performs a vaginal exam and guides the electrode through the cervix to the presenting fetal part, usually the scalp. The electrode is gently attached to the skin according to the device instructions. The wire is connected to the monitor, and the tracing is checked for signal quality. The electrode is removed after delivery or sooner if monitoring changes are needed.

Risks and Precautions

Possible risks include a small scalp mark, bleeding, infection, bruising, or rarely more serious injury. Fetal scalp electrodes are not used in every labor and may be avoided with certain maternal infections, bleeding concerns, or when the fetal presenting part is not accessible. The membranes must usually be ruptured before placement. Fever, abnormal fetal tracing, heavy bleeding, or signs of infection require close obstetric evaluation.
