What Are Colony-Stimulating Factors Used For?

Granulocyte colony-stimulating factors are used to reduce the duration or complications of neutropenia in selected patients receiving myelosuppressive chemotherapy. They are also used in some bone marrow transplantation, stem-cell mobilization, severe chronic neutropenia, or radiation-exposure settings. Granulocyte-macrophage colony-stimulating factor products have narrower and different indications. The product, timing, and treatment goal depend on the clinical situation.

How Do Colony-Stimulating Factors Work?

These medicines bind to specific receptors on hematopoietic progenitor cells and more mature blood cells. Granulocyte colony-stimulating factor signaling promotes neutrophil precursor proliferation, maturation, release, and function. Pegylation can slow drug clearance and extend the dosing interval. The increase in circulating neutrophils develops over time rather than immediately after injection.

How Are Colony-Stimulating Factors Given and Monitored?

Products are administered by subcutaneous injection, intravenous infusion, or specialized delivery devices according to the label. Timing relative to chemotherapy is carefully controlled because administration too close to cytotoxic treatment can be inappropriate. Blood counts and clinical response are monitored to guide duration or dose. Patients using home injections require training in storage, preparation, injection, and disposal.

What Are the Side Effects and Precautions?

Bone pain is common, and headache, injection-site reactions, or fever can also occur. Serious risks include splenic rupture, acute respiratory distress syndrome, severe allergic reactions, capillary leak syndrome, kidney inflammation, and sickle cell crisis. Very high white blood cell counts can occur, and growth-factor receptors have been found on some malignant cells. Left upper abdominal or shoulder pain, breathing difficulty, swelling, or severe weakness requires prompt assessment.
