A calcineurin inhibitor is an immunomodulating medicine that blocks the enzyme calcineurin. Calcineurin normally helps activate T lymphocytes and promotes production of immune-signaling proteins such as interleukin-2. Inhibiting this pathway reduces selected immune responses. Major examples are cyclosporine, tacrolimus, and the topical medicine pimecrolimus.

What Are Calcineurin Inhibitors Used For?

Systemic calcineurin inhibitors are used to prevent rejection after organ transplantation. They are also used for selected autoimmune or inflammatory conditions when their benefits outweigh the risks. Topical tacrolimus and pimecrolimus treat certain cases of atopic dermatitis, while ophthalmic cyclosporine products are used for selected ocular surface conditions. The formulation, dose, and monitoring differ greatly among these uses.

How Do Calcineurin Inhibitors Work?

Cyclosporine binds to cyclophilin, while tacrolimus and pimecrolimus bind to FK506-binding proteins. These drug-protein complexes inhibit calcineurin and prevent activation of nuclear factor of activated T cells. Reduced signaling decreases transcription of interleukin-2 and other cytokines needed for T-cell activation. The result is suppression of immune and inflammatory activity.

How Are Calcineurin Inhibitors Taken or Applied?

Systemic products are available in oral and intravenous formulations, and topical products are applied to the skin or eye according to their labels. Cyclosporine formulations are not always interchangeable because absorption can differ. Tacrolimus and cyclosporine commonly require blood-concentration monitoring after transplantation. Doses are adjusted according to clinical response, kidney function, adverse effects, interactions, and measured drug levels.

What Are the Side Effects and Precautions?

Systemic calcineurin inhibitors can cause kidney injury, high blood pressure, tremor, headache, electrolyte abnormalities, infection, and increased malignancy risk. Tacrolimus can contribute to high blood glucose, while cyclosporine can cause gum overgrowth or increased hair growth. Both medicines have numerous cytochrome P450 3A and transporter interactions, including clinically significant interactions with grapefruit products and selected antifungals or antibiotics. Close laboratory and clinical monitoring is required.

Are calcineurin inhibitors steroids?

No. They suppress immune activity through calcineurin signaling rather than through glucocorticoid receptors. They can be used with or instead of corticosteroids in selected treatment plans.
