A brain mapping system is used when clinicians need to understand where important brain functions are located in an individual patient. It may be used before or during surgery for brain tumors, epilepsy, vascular lesions, or other conditions near functional areas. Mapping can help surgeons choose a safer route and decide how much tissue can be removed. It is also used in research and specialized neurologic testing depending on the technology.

Types of Brain Mapping Systems

Brain mapping can involve several technologies. Functional MRI, magnetoencephalography, and transcranial magnetic stimulation may be used before surgery to estimate functional areas. Intraoperative mapping can use direct electrical stimulation, electrocorticography, neuronavigation, and task-based monitoring during awake or asleep surgery. The system chosen depends on the condition, surgical plan, and brain function being mapped.

How Is Brain Mapping Done?

The process depends on whether mapping is done before surgery or in the operating room. Before surgery, the patient may complete tasks during imaging or stimulation testing so the system can identify active brain regions. During surgery, clinicians may stimulate small brain areas and monitor speech, movement, sensation, or electrical activity. The mapping results are used with imaging, anatomy, and surgical judgment.

Risks and Limitations

Brain mapping can improve planning, but it does not remove all neurologic risk. Direct stimulation can trigger seizures, temporary speech changes, movement changes, or discomfort depending on the procedure.
