Foveal tears are especially relevant when the distal radioulnar joint becomes unstable because the deep TFCC attachment is an important stabilizer. Surgeons assess tear location, tissue quality, ulnar variance, associated fractures, joint stability, and degenerative changes before choosing treatment. Some patients also require ulnar-shortening or another procedure when abnormal ulnar loading contributes to the injury.

How Is Triangular Fibrocartilage Complex Repair Performed?

The surgeon examines the wrist arthroscopically or through an open approach and identifies the location and stability of the TFCC tear. Repairable peripheral tissue can be sutured to the capsule, while a foveal tear can be reattached to the ulna with a suture anchor or transosseous sutures. The distal radioulnar joint is tested during the operation to confirm improved stability. Associated cartilage damage, synovitis, fractures, or ulnar impaction can be treated during the same operation when needed.

What Is Recovery Like After Triangular Fibrocartilage Complex Repair?

The wrist and forearm are immobilized after surgery to protect the repair and limit rotation while the tissue begins to heal. Hand therapy later progresses forearm rotation, wrist motion, grip strength, and functional use in controlled stages. Heavy lifting, forceful gripping, and sports that load or twist the wrist are restricted until healing and stability are adequate. Recovery commonly takes several months, especially after foveal repair or combined ulnar-shortening surgery.

What Are the Risks and Complications?

Possible complications include infection, stiffness, painful scarring, nerve irritation, persistent ulnar-sided pain, and loss of wrist motion. The repaired TFCC can fail to heal or can stretch, leading to recurrent distal radioulnar joint instability. Suture anchors or other fixation can irritate nearby tissues, and scar formation can limit tendon or joint motion. Additional treatment can be needed for recurrent instability, persistent ulnar impaction, or progressive wrist degeneration.
