Mechanism

The reflected light illuminates the structure from behind, creating a contrast effect. This allows the observer to see fine, subtle details, opacities, or defects in the anterior structure as dark areas against a bright orange/red background (the retro-illuminated fundus).

Applications

Crucially used to detect subtle defects in the corneal endothelium, vacuoles in the contact lens, and small pigment or transillumination defects in the iris.

Cataract Observation

It is the primary method used to assess posterior subcapsular cataracts, which appear as dark, granular structures silhouetted against the red reflex.

How is it set up on a slit lamp?

The illumination arm and the microscope arm are focused on two different planes: the light beam is focused on the fundus (or iris), and the microscope is focused on the cornea (or lens).

What is the 'Red Reflex'?

The red reflex is the orange-red glow seen when light is shined into the eye and reflects off the retina. Retro-illumination uses this reflection as its light source.

Can it detect Fuch's Dystrophy?

Yes. Retro-illumination is excellent for visualizing the corneal guttata (deposits on the endothelium) characteristic of Fuch's Endothelial Dystrophy.
