Why Is PCL Reconstruction Performed?

Surgery is considered for symptomatic high-grade PCL tears that cause persistent instability, pain, or functional limitation despite appropriate rehabilitation. It is also commonly performed when a PCL injury occurs with other major knee-ligament injuries. Many isolated lower-grade PCL injuries can be treated without reconstruction. The decision depends on instability, activity demands, chronicity, associated injuries, limb alignment, and the condition of the knee.

How Is PCL Reconstruction Performed?

The surgeon prepares an autograft or allograft tendon and creates bone tunnels or sockets at the anatomic PCL attachment sites in the femur and tibia. The graft is passed through these sites and fixed under controlled tension to reproduce posterior knee stability. A single-bundle reconstruction most commonly restores the larger anterolateral bundle, while double-bundle techniques reconstruct both major functional bundles. Associated ligament, meniscus, or cartilage injuries can be treated during the same operation or in planned stages.

What Is Recovery Like After PCL Reconstruction?

The knee is protected with a brace while early rehabilitation limits forces that pull the tibia backward. Weight-bearing and knee flexion progress according to the reconstruction, graft fixation, and associated procedures. Physical therapy emphasizes quadriceps activation, controlled range of motion, gait, and later strength and sport-specific function. Return to running, cutting, and contact sports generally takes many months and depends on objective recovery milestones.

What Are the Risks and Complications?

Possible complications include infection, blood clots, stiffness, persistent instability, graft stretching, graft failure, and loss of knee motion. Tunnel placement, graft tension, and fixation can affect the final stability of the reconstruction. Injury to nearby nerves or blood vessels is uncommon but possible, particularly during work near the back of the tibia. Some patients continue to have posterior laxity or later develop posttraumatic arthritis despite reconstruction.

Does every PCL tear require reconstruction?

No. Many isolated PCL injuries can be treated with bracing and rehabilitation. Reconstruction is more commonly considered for persistent high-grade instability or multiligament knee injury.
