The "Better One" Test

During this test, the optometrist or ophthalmologist presents a series of lens combinations, asking the patient, "Which is better, one or two?" until the best possible visual acuity is achieved.

Key Outcome

The result is the final prescription for eyeglasses, specifying the sphere (myopia/hyperopia), cylinder (astigmatism), and axis (orientation of astigmatism).

Distinction from Cycloplegic

It is different from Cycloplegic (Wet) Refraction, where drops are used to paralyze the eye's focusing muscle, often used for children to eliminate the effect of accommodation.

How is it different from Autorefraction?

Autorefraction is an objective test (using a machine) that provides a starting point, while manifest refraction is subjective, fine-tuning that measurement based on the patient's perception.

What is the purpose of the Jackson Cross Cylinder?

The Jackson Cross Cylinder is a tool used during manifest refraction specifically to refine the cylinder power and axis of the astigmatism correction.

Why is it called 'manifest'?

It is called manifest because the testing is done without any pharmacological intervention, testing the vision "as it manifests" in the current accommodative state.
