What Is Lateral Epicondyle Release?

Lateral epicondyle release is surgery used to treat persistent lateral elbow tendinopathy, commonly called tennis elbow. The operation targets diseased tissue near the common extensor tendon origin on the outside of the elbow, especially the extensor carpi radialis brevis. Depending on the technique, the surgeon releases or debrides abnormal tendon tissue and can repair the remaining tendon back to bone. The procedure can be performed through an open, arthroscopic, or percutaneous approach.

Why Is Lateral Epicondyle Release Performed?

Surgery is reserved for selected patients whose lateral elbow pain remains functionally limiting despite a prolonged course of nonsurgical treatment. Clinicians first confirm that symptoms are consistent with lateral elbow tendinopathy rather than nerve compression, arthritis, instability, or another cause. Most patients improve without surgery, and evidence has not shown surgery to be clearly superior to continued nonoperative care for every chronic case. The decision therefore depends on symptom severity, tendon findings, duration, functional demands, and response to prior treatment.

How Is Lateral Epicondyle Release Performed?

The surgeon identifies the diseased portion of the common extensor origin at the lateral epicondyle. Degenerated tendon tissue is released or removed while healthy tendon, nearby nerves, and the lateral collateral ligament complex are protected. The underlying bone can be lightly prepared to create a healing surface, and the remaining tendon can be repaired with sutures or an anchor when needed. Arthroscopic techniques can also evaluate and treat selected problems inside the elbow joint during the same operation.

What Is Recovery Like After Lateral Epicondyle Release?

The elbow is protected briefly after surgery while early range of motion is introduced to reduce stiffness. Lifting, gripping, and resisted wrist extension are limited until the tendon and incision have healed adequately. Physical therapy can progress motion, forearm strength, and gradual return to work or sports. Recovery usually takes several months before forceful repetitive activity is resumed without restriction.

What Are the Risks and Complications?

Possible complications include infection, bleeding, stiffness, painful scarring, persistent pain, and injury to nearby nerves. Excessive dissection near the lateral ligament complex can contribute to posterolateral rotatory instability of the elbow. Symptoms can continue when the original diagnosis was incorrect, diseased tendon remains, or another source of pain is present. Revision surgery can be needed for persistent pain, instability, or another complication.
