How Does Eplerenone Block Aldosterone?

Eplerenone binds mineralocorticoid receptors and blocks aldosterone binding. Aldosterone is part of the renin-angiotensin-aldosterone system and promotes sodium retention and potassium excretion. Blocking its receptor changes sodium and potassium handling and contributes to lower blood pressure and heart failure treatment after myocardial infarction.

Who Is Treated With Eplerenone?

Eplerenone is used in stable adults with symptomatic heart failure and left ventricular ejection fraction of 40 percent or less after acute myocardial infarction. It is also approved for adult hypertension. Serum potassium and kidney function are assessed before therapy and during treatment.

How Is Eplerenone Dosed?

Post-myocardial-infarction heart failure starts at 25 mg once daily and is titrated toward 50 mg once daily within four weeks as tolerated. Hypertension starts at 50 mg once daily and can increase to 50 mg twice daily when blood pressure response is inadequate. Potassium levels guide dose changes.

What Serious Risks Are Linked to Eplerenone?

Hyperkalemia is the main serious risk and is more common with reduced kidney function, diabetes, proteinuria, or interacting medicines. Eplerenone is contraindicated when serum potassium exceeds 5.5 mEq per L at initiation, when creatinine clearance is 30 mL per minute or lower, and with strong CYP3A inhibitors.
