How Does Epidiolex Affect Seizures?

The precise anticonvulsant mechanism of cannabidiol in humans is unknown. Current labeling states that its seizure-reducing effects do not appear to result from interaction with cannabinoid receptors. Cannabidiol is metabolized mainly in the liver and has several clinically relevant drug interactions.

Who Is Treated With Epidiolex?

Epidiolex treats seizures associated with Lennox-Gastaut syndrome, Dravet syndrome, and tuberous sclerosis complex in patients age 1 and older. Liver transaminases and total bilirubin are checked before therapy starts. Dose adjustments are used in moderate or severe liver impairment.

How Is Epidiolex Dosed?

For Lennox-Gastaut syndrome or Dravet syndrome, treatment starts at 2.5 mg per kg twice daily and rises after one week to 5 mg per kg twice daily. The dose can rise to 10 mg per kg twice daily. Tuberous sclerosis complex starts at the same dose and titrates to 12.5 mg per kg twice daily.

What Serious Risks Are Linked to Epidiolex?

Serious concerns include liver injury, sleepiness and sedation, suicidal thoughts or behavior, hypersensitivity reactions, and increased seizure frequency or status epilepticus after abrupt withdrawal. Liver injury risk rises with higher Epidiolex doses and with valproate. Clobazam can also increase sedation and influence liver-related risk.
