How Does Enbrel Block TNF?

Etanercept is a soluble form of the p75 tumor necrosis factor receptor. It binds TNF-alpha and TNF-beta and prevents these inflammatory cytokines from binding to cell-surface TNF receptors. Reduced TNF signaling decreases inflammatory activity involved in several joint and skin diseases.

Which Conditions Are Treated With Enbrel?

Enbrel treats moderate to severe rheumatoid arthritis, psoriatic arthritis, active ankylosing spondylitis, and chronic moderate to severe plaque psoriasis in adults. Pediatric uses include moderate to severe polyarticular juvenile idiopathic arthritis from age 2, juvenile psoriatic arthritis from age 2, and plaque psoriasis from age 4.

How Is Enbrel Dosed?

Adults with rheumatoid arthritis, psoriatic arthritis, or ankylosing spondylitis receive 50 mg subcutaneously once weekly. Adult plaque psoriasis starts at 50 mg twice weekly for three months, followed by 50 mg once weekly. Pediatric pJIA, plaque psoriasis, and juvenile psoriatic arthritis use 0.8 mg per kg weekly up to 50 mg.

What Serious Risks Are Linked to Enbrel?

Enbrel carries a boxed warning for serious infections and malignancies. Reported infections include tuberculosis, bacterial sepsis, invasive fungal infections, and other opportunistic infections. Other serious concerns include demyelinating disease, new or worsening heart failure, blood-cell disorders, hepatitis B reactivation, severe allergic reactions, and autoimmune hepatitis. Sepsis is a contraindication.
